Mark your calendars! On August 10th we will have a mandatory parents meeting. Because of the many changes this upcoming year holds, we insist that all parents (returners too!) attend. At this meeting we will collect the following completed forms:
- Child Enrollment Form
- Medical Care Plan (if there is a special medical concern or issue)
- Child Medical Statement
- COPA Application
- UPK Consent Form
- Family Questionnaire
- Family Survey
- Photo Consent
You can download these forms here and bring them to the parents meeting completed. If you plan on filling out the forms at the meeting, please bring your, your spouses (if applicable), and your child’s social security numbers.
Everyone is also required to bring a copy of your most recent tax return (first page only). If you are unable to make a copy you may use the preschool’s copy machine. The office will be open for copying tax returns from 3:30- 4:00.
There are a lot of forms so please take your time and be sure to fill out each line (including NA when necessary). No fields should be left blank. Only one form needs to be signed and dated by your child’s physician. Please take this form (Child Medical Statement) with you to your appointment. If your child has been to the doctor within the past year you can contact them and request they sign our form with the information from your child’s last visit. All medical statements are good for 12 months. After the 12 months have passed, a new medical statement must be completed by your child’s physician and returned to Parma Preschool. Without a current medical statement, your child is ineligible to attend school.
You’ll notice that the Child Enrollment Form asks you to sign after you’ve received a copy of the school’s handbook. Our newly updated handbook will be available online by August 1, 2010. Posting it on our website allows us to save paper! If you do not have access to the internet we can provide you will a copy at the parents meeting.
We would also like to remind you that tuition is due on the first of each month and payment is made one month in advanced. To secure your child’s spot at Parma Preschool we expect your first tuition payment to be made by August 1. October's tuition is due by September 1. Either of these payments can be made at our parents meeting on August 10th.
